Accident reporting in the workplace must be done to prevent further accidents from happening and to protect the employer and the employee in the event the employ must receive medical treatment. Describe the direct and indirect causes of the accident. Was there an unsafe act being performed or was the workplace conditions responsible for the accident. Analysis why and how the accident happened giving details about any environmental or personal factors that played a part in the accident.
Prevention Recommendations
Detail any ways that this type of accident could be avoided in the future. Detail the basic cause of the accident and how things could change to prevent the same thing from happening again. Give details about any protective gear or equipment that may be needed to prevent this accident from happening to someone else. Note if workplace training could help avoid this accident in the future or different workplace policies.
