Accident reporting in the workplace must be done to prevent further accidents from happening and to protect the employer and the employee in the event the employ must receive medical treatment. If the accident is severe, OSHA will investigate and use the accident report to aid in the investigation. Accident reports must be accurate to help.

Anyone that has been involved in a workplace accident must report the incident to his or her supervisor. An accident report will be filled out and kept on file. If you go to the doctor, this report is kept with your documents from a medical professional. If you don't seek treatment, the incident report is kept on file in case you must seek medical attention in the future.

The information supplied on the accident report must be detailed. It should describe where the accident happened and what time the accident occurred. An employee must report who was present when the accident happened and what was involved in the accident. Detailed information must be accurate.

A description of what lead to the events surrounding the accident must be reported. Give a step-by-step description of how the accident happened and the type of damage that occurred. Give details about what parts of the body if any were affected by the accident. Describe if any hazardous materials were involved and what they were.

Discuss the Accident

Describe the direct and indirect causes of the accident. Was there an unsafe act being performed or was the workplace conditions responsible for the accident. Analysis why and how the accident happened giving details about any environmental or personal factors that played a part in the accident.

Prevention Recommendations

Detail any ways that this type of accident could be avoided in the future. Detail the basic cause of the accident and how things could change to prevent the same thing from happening again. Give details about any protective gear or equipment that may be needed to prevent this accident from happening to someone else. Note if workplace training could help avoid this accident in the future or different workplace policies.
